DO I NEED A REFERRAL?

In many cases, you can schedule directly with our orthopedic team without a referral. If your insurance requires one, your primary care provider can send it to us. Not sure? Call us at the numbers below and we will help you figure it out.

WHAT TO BRING TO YOUR FIRST VISIT

  • A list of your current medications
  • Any prior imaging you have (X-rays, MRI scans)
  • Your insurance card and photo ID
  • A list of questions or concerns you would like to discuss

 

WHAT HAPPENS AFTER I SCHEDULE AN APPOINTMENT?

After you schedule, your care team will confirm your appointment and let you know if any forms or imaging need to be completed in advance. At your first visit, your provider will evaluate your symptoms and discuss your options, whether that involves non-surgical treatment, further imaging or a surgical consultation.
