Discharge Planning and Social Services

Ongoing care may be necessary after you leave the hospital. You will be discharged to the "next level of care" for healing, recuperation, and rehabilitation depending on your needs. The Discharge Services staff may assist you and your family in your transition from the hospital. Discharge Services are free and available to all Wood County Hospital patients and families. Our goal is to assist you in developing a safe and successful discharge plan and provide you and your family with support during your hospital stay.

There are a variety of discharge plans available based on the needs of you, the patient. Examples include going home with home health care services, transfer to a skilled nursing facility for short term rehabilitation, and referral to community resources when needed.

Discharge Services staff will also provide

  • Emotional support to patients and families addressing end of life issues
  • Advance Directives information
  • Information on community resources for continuation of care

You can also request a referral for Discharge Services from your nurse. You can also call the Licensed Social Worker directly at (419) 354-8980 or a RN Discharge Planner at (419) 354-8980 or (419) 373-4177.

Discharge

Discharge authorization must come from the physicians involved in your care. When you are discharged, please try to make arrangements as soon as possible. Staff will assist you while exiting at discharge. It is best to have a family member or friend accompany you after discharge to help with your transition back to home.
