Diabetes Education Program

Diabetes is a complex disease that can be challenging to manage on your own. Diabetes self-management training can help you manage your diabetes and live a healthier life.

Our outpatient program provides diabetes self-management training taught by certified diabetes care and education specialists and registered dietitians.

Our program is recognized by the American Diabetes Association for meeting national diabetes education standards. 

All diabetes self-management training requires a physician referral.  As a courtesy, we will verify your insurance coverage prior to your first appointment.  We may ask you to check your coverage if your insurance coverage will not provide the information to us.  Medicare, Medicaid and many private insurance companies cover for diabetes self-management training.

Individual appointments and classes are offered at Wood County Hospital. In addition, individual appointments are also available at Falcon Health, Bowling Green Clinic, Bowling Green Family Care Center, Deshler Family Practice and North Baltimore Family Practice. Diabetes Self-Management Training 

Individual Diabetes Assessment (approximately 1 hour)

Class Series - Morning and Evening sessions are offered in a 2 or 3 class series for a total of 6 hours of teaching

The following topics are covered in these sessions:

    • Types of diabetes, insulin resistance and pre-diabetes
    • Blood sugar goals and using a glucometer
    • Recognizing and treating high and low blood sugar
    • Review goals for blood pressure and lipids
    • Diabetes medications
    • Nutrition and Meal planning
    • Physical activity and it’s effect on blood sugar
    • Stress, Emotions and Diabetes
    • Support and Community Resources
    • Prevention of diabetes complications
    • Goal setting for self-management of diabetes

 

Follow-up Class or Individual Appointment (approximately 1-1 ½ hours)

This session is typically about 4-6 months after the initial assessment and covers the following:

    • Staying motivated
    • Roadblocks to success
    • Review of goals set in the initial class series
    • Review of blood sugar and meal logs and new labs if available

 

Follow-up Class or Individual Appointment (approximately 1-1 ½ hours)

This session is typically about 1 year after your initial assessment and covers the following:

    • Changes in diabetes care and treatment
    • Knowing your numbers 
    • Staying the course

Individual Diabetes Self-Management Training 

Diabetes Self-Management Training may be provided on an individual basis based on one’s individual needs and situation. 

    • Insulin initiation skills session
    • Gestational diabetes
    • Refresher diabetes education for those who have already attended Diabetes Self-Management Training that need additional assistance with controlling their diabetes.
