What’s the benefit of using MyHealthRecord?
MyHealthRecord allows you to access your personal health information from any computer or mobile device, easily, immediately and accurately.
How do I sign up to use MyHealthRecord?
Self-enroll: Click Self Enrollment to self-enroll if you have an email address on file:
Registration: Make sure the registration representative enters your email and sends you an invite to access MyHealthRecord.
Contact Us: Call the COVID Patient Portal Support Line (419) 728-0642 to register your email and receive an invite to access MyHealthRecord.
Who can sign up for MyHealthRecord?
Adults, 18 years and over, are eligible to set up an account.
Proxy access is available for those minor patients age 0-12 contact MyHealthRecord patient portal support line at 419-728-0642 for assistance in enrollment. (Proxy access is automatically terminated when minor reaches age 18.)
Proxy for minors age 12-18, will require their own email address in order to be enrolled. Proxies can be established for patients in this range, however, there will be limited information that is available to the proxy.
Can I enroll my spouse or another family member in MyHealthRecord?
To maintain HIPPA compliance, each patient will need to enroll themselves by one of methods above. A patient can grant proxy access to other people.
How or where do I access MyHealthRecord?
Is my information secure?
Yes, MyHealthRecord is HIPPA compliant and provides you with the ability to securely view your health information.
What information can I access?
Using your secure password, you can view your upcoming appointment details, lab results, view hospital discharge documents, current and active mediations, allergies, and past medical procedures.
What Results Can I See on MyHealthRecord?
Most clinical results may be viewed from anywhere there is internet access. Certain test results such as HIV, STD, or cancer-related conditions, as well as Pathology, Microbiology and Blood Bank are not provided in MyHealthRecord. Results are show in real time following the completion of the test.
How far back can I access my information?
Medical information prior to Dec 5, 2011 is stored in your electronic medical record, but may not be available in MyHealthRecord.
Why is Cerner referenced on the website?
Cerner is the company we have partnered with to manage your electronic medical record information. MyHealthRecord is a customized Cerner system that provides you with a view into your electronic medical record.
What if I didn’t receive my invitation email or forgot to accept the invitation before the deadline?
