Diabetes Education Program

Diabetes management education, medication administration education is provided by registered nurses and registered dietitians specially trained in diabetes management. The Diabetes Education Program assists patients in diabetes self-management and continuing education for the diabetic population of the surrounding area. 

 

The Diabetes Education program is recognized by the American Diabetes Association (www.diabetes.org)  (Medicare Certified).  Patients are instructed on modified diets, blood glucose monitoring, management tools, nutrition and meal planning, physical activity planning, medications, self-care do’s and don'ts, stress management, and mental wellness. Education is also available for pregnant women with gestational diabetes.

 

The health care team includes a physician, nurse educator and dietitian.  The keys to good diabetes management are enlisting the help of your team and working together to reach your goals.

 

A physician referral is required, which can be faxed to (419) 354-8682. Many insurances cover the cost of education and patient counseling. Patients are asked to contact their plans to determine coverage and whether prior authorization is required.

 

Diabetes Interest Group

 

The Diabetes Education Program at Wood County Hospital is hosting a four-part series this fall on food and nutrition as part of its Diabetes Interest Group. Each session will cover current topics and information about diabetes including special recipes, taste testing, and food demonstrations.   Dates and topics include:

  • Thursday, December 10th, 12:30 – 1:30 p.m.
    • New Information on Sweeteners
  • Thursday, January 14th, 12:30- 1:30 p.m
    • Brown Bag Day: Medication Safety Review
  • Thursday February 11th, 12:30-1:30 p.m

  • April, Date to be Announced
    • Diabetes Health Fair- Topic to be Announced
  •  Thursday May 13th, 12:30-1:30 p.m
    • Grocery Store Tour

The Diabetes Interest Group is free and open to anyone interested in learning more about diabetes. Free blood glucose screenings are also available at the meetings. Lunch may be purchased for $2 in the cafeteria for those attending the programs, or attendees may bring their own lunch. The programs are held in the WCH meeting rooms.

The program consists of a one hour assessment and a series of classes.  The classes are offered both during daytime and evening hours.

Fee Structure:           $40  per half hour for individual sessions

                                   $23 per half hour for group sessions
