Discharge Planning and Social Services

It is not uncommon to need ongoing care after you leave the hospital. You will be discharged to the “next level of care” for healing, recuperation and rehabilitation depending on your needs. The Discharge Services staff is here to assist you and your family plan your transition from the hospital. Service is free and available to all WCH patients and families. Our goal is to assist in developing a safe and successful discharge plan and provide you and your family needed support during your hospital stay.

Typical discharge plans include: going home with home health care services, transfer to a skilled nursing facility for short term rehabilitation or other special care, home medical equipment and referral to community resources when needed. Staff also provides emotional support for patients and families addressing end of life issues; provision of Advance Directives information; and linkage to community resources for continuation of care.
